DBQ evaluations are priced at $350 per condition. Nexus Letter pricing is based on the number of conditions evaluated. Additional DBQs, medical-record reviews or documentation outside the selected service may require separate pricing.

What to bring to your
veteran evaluation
Bringing complete and relevant medical documentation helps your provider conduct a thorough independent evaluation. Veterans traveling from San Antonio, Garden Ridge and surrounding South Texas communities should prepare these records before their appointment whenever possible.
VA Medical Records
Bring relevant VA treatment notes, diagnoses, medication history and records documenting the symptoms or conditions being evaluated.
Service Treatment Records
Include available medical documentation from active-duty service, military clinics, deployment records or documented in-service injuries.
Prior VA Decisions
Bring previous rating decisions, denial letters, decision explanations or other correspondence related to your disability claim.
MRI, CT & X-Ray Reports
